There is a new screening tool that can help OSF HealthCare detect any patient in the ten different areas of social needs such as food insecurity, housing, intimate partner violence and more. The new screen and connect tool was able to indicate a patient who came in with stomach issues was at high risk for financial, housing and food insecurity.

He had eaten a piece of spoiled meat because he was starving and that was all he had. Physician Assistant Mandy Robinson tells us how they were able to provide the patient with help.

The OSF Innovation team, medical group doctors, and care team members worked together to come up with the screening tool while working in a rural community. They found that this tool would make it easy to access data and learn more about the challenges their patients were facing.
